About this role

Tufts Medicine Care at Home is seeking a full-time Nurse Practitioner to join our team in the greater Malden area, primarily providing home-based palliative care with occasional virtual hospice coverage. This role focuses on delivering compassionate, expert care to patients with serious or life-limiting illnesses in their own homes.

In this role, you will conduct patient visits to manage pain and symptoms, facilitate goals-of-care discussions, and maintain thorough documentation of each encounter. You will collaborate closely with physicians board-certified in hospice and palliative care, as well as social workers skilled in addressing the physical, emotional, and spiritual challenges of serious illness.

Requirements

  • Graduate of an accredited and/or approved nurse practitioner program
  • Current license as a Registered Nurse and Nurse Practitioner in the Commonwealth of Massachusetts
  • Extensive knowledge, experience, and expertise in hospice and palliative care
  • Interpersonal skills to establish and maintain relationships within the medical community
  • Demonstrated ability to work as a team player and function as a member of an interdisciplinary team
  • A team player who is excited to work cooperatively with a wide range of stakeholders and disciplines to champion world-class patient satisfaction and support

Responsibilities

  • Remain current on clinical standards of practice in hospice and palliative care and promote evidence-based practice
  • Provide medical expertise on pain and symptom management for patients with serious or life-limiting illness
  • Facilitate discussions on goals of care with all patients and their families
  • Identify and evaluate clinical issues, reach sound conclusions, generate alternatives, and make accurate and timely decisions
  • Maintain documentation of patient visits and communicate recommendations to colleagues
  • Utilize interpersonal skills to establish and maintain relationships within the medical community
  • Provide occasional virtual hospice coverage as needed
