As a Wound Care Nurse with Esperta Health, you will deliver expert wound care to patients in the comfort and safety of their homes. You will be responsible for assessing and treating wounds according to established wound healing guidelines, using an application-based electronic patient record.

Collaboration with the interdisciplinary team, thorough documentation, and ongoing patient education are essential aspects of this role. You will carry out wound care procedures within the nursing practice scope, including applying dressings and other related treatments, while following infection control protocols and prioritizing patient safety.

This role offers the opportunity to work independently while being part of a multidisciplinary team dedicated to improving patient outcomes. You will stay current with HIPAA regulations and have access to training and development to advance your wound care expertise.

Requirements

  • Licensed Registered Nurse (RN) with an active and unrestricted nursing license in the state of practice.
  • Bachelor of Science in Nursing (BSN) degree preferred.
  • Minimum of three years of experience in wound care nursing preferred.
  • Certification in Wound, Ostomy, and Continence Nursing (WOCN) or other company-approved wound care certification is a plus; new hires may be eligible to take the certification exam within 60 days of employment with reimbursement upon passing.
  • Strong knowledge of evidence-based wound care practices and guidelines.
  • Proficient in documentation using electronic health record (EHR) systems.
  • Valid driver’s license and reliable transportation, with ability to travel to patient homes.
  • Commitment to maintaining patient confidentiality and compliance with HIPAA regulations.

Responsibilities

  • Perform thorough wound assessments and physical examinations in patients' homes.
  • Administer wound care treatments based on evidence-based Wound Healing Guidelines (WHG) and create individualized care plans.
  • Keep accurate and prompt electronic health records in accordance with regulatory requirements.
  • Provide education to patients and caregivers on wound care techniques and prevention methods.
  • Carry out wound care procedures within the nursing practice scope or under supervision, including applying dressings and other related treatments.
  • Follow infection control protocols and prioritize patient safety during all care activities.
  • Exhibit strong communication and customer service skills to effectively address patient and caregiver concerns.
  • Stay current with HIPAA Privacy, Security, and Breach Notification regulations.
