About this role
Since 2012, Giving Home Health Care has supported individuals impacted by health conditions related to their work in nuclear facilities for the Department of Energy. We focus on personalized, in-home care for patients across nine states, including New Mexico.
This 1099 contract role in Caballo, NM, involves providing care at the patient’s residence for approximately 8 hours per month, with flexible scheduling around the patient’s preferences. Duties include executing physician-prescribed care plans, managing medications, monitoring vitals, and assessing skin integrity.
You will work one-on-one with the patient, collaborating closely with CNAs, HHAs, case managers, and physicians. The role emphasizes advocacy and building lasting relationships, as there is no expiration for the care benefit once a patient is accepted into the program.

Requirements
- Graduate of an accredited college or university, Associate or Baccalaureate School of Nursing.
- Current State or Compact License as a Registered Nurse or Licensed Practical Nurse.
- Current CPR certification.
- TB test and physical examination.
- Maintain required insurance.
- Hand-eye coordination and manual dexterity.
- Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients.
- Must have dependable transportation and be willing to commute to the patient’s home.
Responsibilities
- Execute physician prescribed plans of care.
- Complete compliant documentation of the care provided in the system of record.
- Manage and administer prescribed medication, treatment, and therapies.
- Conduct patient assessments and coordination of care.
- Monitor vitals and GI intake and output.
- Assess skin integrity and administer wound care.
- Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression.
- Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients.
