About this role

As a Maternal Child Health Nurse at Dartmouth Health Home Care, you will manage a case load of antenatal, postpartum, newborn, and pediatric home care patients. You will complete the nursing process using a family-centered and developmentally appropriate approach.

In your day-to-day role, you will develop a plan of care under the direction of the patient’s physician and in collaboration with the patient and family. You will provide ongoing assessment, implement nursing interventions, and educate patients and caregivers on their health needs.

You will be part of a cohesive interdisciplinary team that collaborates with health care providers and community services to ensure patient needs are met. Dartmouth Health Home Care covers over 70 towns, delivering superior nursing and home health services with integrity and compassion.

Requirements

  • Licensed Registered Nurse in Vermont and New Hampshire, or hold a compact license.
  • Graduate of an accredited school of nursing.
  • One year of experience as an RN in maternal or pediatric home health care setting preferred.
  • Valid driver’s license.
  • Current car insurance meeting minimum standards.
  • BLS certification within 90 days of hire.

Responsibilities

  • Develop a plan of care under the direction of the patient’s physician and in collaboration with the patient, family, and primary caregivers upon admission.
  • Initiate referrals for interdisciplinary services and communicate with other disciplines in a timely manner.
  • Provide ongoing assessment of the patient’s clinical, functional, and mental status to identify problems and progress toward desired goals.
  • Follow agency policy when mandatory reporting is required.
  • Complete nursing interventions and education relevant to the client, reinforcing education provided by other health care providers.
  • Accept responsibility for assignment to perform specialized procedures and see patients in other programs such as Home Health and Hospice.
  • Provide clinical oversight of LPNs and LNAs in the home.
