At CVS Health, we are building a world of health around every individual. Signify Health seeks a Per Diem Clinician to provide In-Home Health Evaluations, engaging people in their homes to bridge gaps in care.
The 45-minute visit allows time to connect one-on-one and answer health questions. It includes medication and medical history review, physical evaluation, and additional diagnostic tests as ordered. You'll use an iPad with an intuitive clinical workflow.
You'll bring your heart into every visit as part of a national network of purpose-driven clinicians dedicated to improving health outcomes. Your primary workplace is the private residence of health plan members, which may present diverse conditions including household pets and secondhand smoke.
You play a critical role in building trusted relationships to make people healthier. Many clinicians enjoy the freedom to focus on the clinical part of their job without administrative responsibilities. You help guide people to better care and make a change in communities.
Requirements
- Active and unrestricted Nurse Practitioner or Physician Assistant license in the state of practice.
- National board certification as a Nurse Practitioner (AANP/ANCC) or Physician Assistant (NCCPA).
- Ability to travel locally and within licensed states as needed.
- Comfortable working in diverse home environments including those with household pets and secondhand smoke.
- Proficient with tablet technology and electronic clinical documentation.
- Valid driver's license and reliable transportation.
Responsibilities
- Bring your heart into every visit.
- Conduct 45-minute in-home health evaluations including medication and medical history review, physical evaluation, and ordered diagnostic tests.
- Use an iPad and an intuitive clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services including Diagnostic and Preventive Services products such as spirometry, diabetic eye exams, and bone density screening.
