About this role
Vancouver Coastal Health (VCH) is seeking a Rural Outpost Nurse / Community Health Nurse (RN) to join the team at the Private Clinics Health Centre in Gillies Bay, BC. This role works within a client and family centered care model, serving acute, chronic, and palliative populations in the community.
In this position, you will assess service eligibility, develop and adapt care plans, deliver direct care, and provide care management to help clients navigate available services. You will work in partnership with clients and families to establish trauma-informed, safe, and realistic goals.
You will collaborate with an interprofessional team, including the primary care provider and other care partners, to coordinate and transition nursing and other services. The role incorporates current evidence into practice and operates within the vision and values of VCH.

Requirements
- Registration with BC College of Nurses and Midwives as a practicing RN(C) registrant.
- Remote Nursing Certification or eligible for Remote Nursing Certification to be obtained within one year of hire, OR three years' recent related experience including Emergency training or equivalent.
- Basic Life Support, Level 'C' certificate.
- Advanced Cardiac Life Support (ACLS).
- Valid BC driver's license.
- Knowledge of available programs and services, their interrelationships, and their function in delivering care in the community.
- Demonstrated knowledge of the BCCNM Professional and Practice standards for nursing.
- Knowledge of the Community Health Nurse of Canada (CHNC) competencies and standards of practice.
Responsibilities
- Work within a client and family centered care model with acute, chronic, and palliative populations in the community.
- Assess service eligibility and facilitate the development, implementation, and adaptation of a care plan.
- Deliver direct care and provide care management to assist clients in managing their own care and navigating community services.
- Partner with clients and families to establish trauma-informed, safe, realistic, and reasonable goals.
- Coordinate, organize, implement, evaluate, and transition nursing and other services in an interprofessional team model.
- Collaborate with the primary care provider and other care providers to ensure integrated care.
- Incorporate current evidence into practice and consult with other system partners as needed.
