As a Visit Nurse, you will assess and identify patient/family needs and plan, implement, evaluate, and document individualized hospice nursing care. You will coordinate care among the interdisciplinary group and other medical providers while supervising hospice care provided by LPNs and CNAs.

Your days may include making visits to patients in home, skilled nursing facilities, assisted living, or acute care settings. You will also conduct admission visits for medically eligible patients and support unplanned visits as needed to meet team coverage needs.

This weekend role offers a Friday–Sunday schedule from 8:30 AM to 9:00 PM with weekend premium pay. You will join a team empowered to provide care described as above and beyond, enriching both patients’ lives and the community.

Requirements

RN with current licensure in Kentucky and/or Indiana or ability to obtain required license upon offer.
BSN desired.
Certification in hospice/palliative care preferred.
One year of nursing experience preferred.
Knowledge of palliative care helpful.
Valid driver’s license required.
Use of technology-based tools, including electronic medical record, strongly preferred.

Responsibilities

Assess and identify patient/family needs to plan, implement, evaluate, and document individualized hospice nursing care.
Coordinate care and collaborate among the Interdisciplinary Group and other medical providers.
Supervise hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants.
Make visits to patients in home, skilled nursing facility, assisted living facility, or acute care settings.
Conduct admission visits for medically eligible patients into Hosparus Health care.
Support unplanned visits and follow-up on new admissions as needed for team nurse coverage.
