About this role

The Respond Healthcare employed Community Stoma Care Nurse will be dedicated to developing the Stoma Care Nursing service across the Birmingham community and surrounding areas. This role provides support to acute hospitals to generate referrals to the Respond service. The nurse delivers professional and ethical nursing care while working in partnership with key stakeholders.

The CSCN supports new patients referred onto the Respond Dispensing Appliance Contractor service from hospitals across the Birmingham regions. Duties include easing smooth transitions from hospital to community and performing home visits in line with agreed patient referral pathway models. The role involves managing patient care following discharge from hospital.

Working autonomously in the community requires self-motivation and drive to develop the service. The nurse combines commercial awareness with expertise and advanced clinical judgement. Employment is with Respond Healthcare Limited reporting directly to the Nurse Team Leader.

Through this service the nurse contributes to the growth and success of Respond Healthcare by developing an ethical Stoma Care Nursing Service. Support is provided to NHS acute services where community support is required. The position extends the acute based teams into the community setting.

Requirements

  • Registered Nurse with current NMC registration
  • Specialist experience in stoma care nursing
  • Ability to work autonomously in community settings
  • Advanced clinical judgement in patient assessment and care planning
  • Commercial awareness applied within ethical professional nursing practices
  • Self-motivated with drive to develop a community nursing service

Responsibilities

  • Collaborate with NHS Trust Stoma care Teams to support patient discharges onto Respond following surgery
  • Work as an extension of the acute stoma care team and support introduction of the patient pathway of care within the community
  • Provide professional and ethical nursing care services to patients by working in partnership with key stakeholders
  • Perform home visits in line with agreed patient referral pathway models
  • Develop community service with NHS Trusts and introduce the Respond DAC service to patients in the community
  • Utilise commercial awareness in the use of group medical appliances and ancillary products where suitable
  • Ease a smooth transition for patients from hospital to the community and manage care following discharge
  • Support acute hospitals to generate referrals to the Respond service
