This role supports the Atrium Health Wake Patient Access and Care Team (PACT) as a Telehealth Nurse, providing remote clinical triage and care coordination for patients in the Winston-Salem area. You will work 20 hours per week, primarily during evening shifts, with a required rotation of every other weekend and holidays.

As a key member of the PACT team, you will conduct telephonic patient assessments, guide individuals to appropriate care settings, and facilitate seamless access to healthcare services. Your work ensures that patients receive timely, expert guidance while reducing unnecessary emergency department visits.

This part-time position is based within the 12707 Enterprise Corporate - Patient Access & Care Team: NC/GA Nursing division, offering a structured yet flexible remote work environment. You will collaborate with a dedicated team of nurses and care coordinators focused on optimizing patient outcomes and operational efficiency.

Requirements

  • Active and unrestricted Registered Nurse (RN) license in North Carolina or Georgia
  • Minimum of one year of clinical nursing experience, preferably in telehealth, triage, or patient access
  • Strong clinical assessment and decision-making skills for remote patient evaluation
  • Proficiency with electronic health records (EHR) and telehealth platforms
  • Ability to work independently in a remote setting while maintaining team collaboration
  • Excellent verbal communication skills for clear patient interaction over the phone
  • Availability for evening shifts and commitment to weekend and holiday rotation

Responsibilities

  • Conduct telephonic patient assessments and triage to determine appropriate care pathways
  • Provide clinical guidance and support to patients accessing healthcare services remotely
  • Coordinate care transitions and referrals within the Atrium Health Wake network
  • Document patient interactions and clinical decisions accurately in the electronic health record
  • Participate in required every other weekend and holiday rotation schedules
  • Collaborate with the Patient Access and Care Team to optimize patient flow and access
  • Adhere to telehealth protocols and nursing standards for remote patient care
