Requirements

  • Current Registered Nurse (RN) license in the state of Illinois
  • Experience in hospice or end-of-life care preferred
  • Ability to travel to patient homes across the Northwest suburbs
  • Strong clinical assessment and critical thinking skills
  • Valid driver's license and reliable transportation
  • Comfortable working independently in a field-based setting
  • CPR certification required

Responsibilities

  • Provide hospice nursing care to patients in the Northwest suburbs based on patient needs
  • Travel to patient homes for scheduled visits during 12-hour day shifts (8am-8pm)
  • Develop and implement individualized care plans in coordination with the hospice team
  • Administer medications and manage symptoms to ensure patient comfort
  • Provide emotional support and education to patients and their families
  • Document patient assessments and care interventions accurately
  • Participate in every other weekend rotation and holiday rotation as scheduled
