About this role
This part-time role at Advocate Aurora Health involves providing compassionate hospice nursing care to patients in the Lake Zurich area. As a Hospice Visit Nurse, you will travel to patient homes across the Northwest suburbs, delivering skilled nursing support and symptom management during 12-hour day shifts.
Your daily responsibilities include coordinating patient care plans, administering medications, and offering emotional support to patients and their families. The schedule follows an 8am-8pm pattern with every other weekend rotation and holiday rotation, ensuring consistent coverage for those in need.
You will be part of a dedicated hospice team focused on comfort and quality of life for patients at the end of life. This role offers the autonomy of field-based nursing while being backed by one of the largest nonprofit health systems in the region.

Requirements
- Current Registered Nurse (RN) license in the state of Illinois
- Experience in hospice or end-of-life care preferred
- Ability to travel to patient homes across the Northwest suburbs
- Strong clinical assessment and critical thinking skills
- Valid driver's license and reliable transportation
- Comfortable working independently in a field-based setting
- CPR certification required
Responsibilities
- Provide hospice nursing care to patients in the Northwest suburbs based on patient needs
- Travel to patient homes for scheduled visits during 12-hour day shifts (8am-8pm)
- Develop and implement individualized care plans in coordination with the hospice team
- Administer medications and manage symptoms to ensure patient comfort
- Provide emotional support and education to patients and their families
- Document patient assessments and care interventions accurately
- Participate in every other weekend rotation and holiday rotation as scheduled
