About this role

This Primary Care Registered Nurse role with Vancouver Coastal Health triages, assesses, plans, implements, and evaluates nursing care for patients and families requiring high priority care within 24 hours. The position operates within a team-based model of care, applying chronic disease management, palliative, health management, and self-care management principles.

Day-to-day, the nurse uses approved triage standards and critical thinking skills to conduct assessments, identify problems, provide direct care, and address variances from the plan of care. The role also involves developing and maintaining partnerships with primary care providers in accordance with the local Community of Care service delivery model and primary care network.

The nurse acts as a resource for the interdisciplinary team, providing information based on nursing theory and practice related to patients with primary level care needs.

Requirements

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
Two years' recent clinical experience relevant to the designated clinical area, or an equivalent combination of education, training, and experience
Demonstrated ability in applying critical thinking skills within safe limits of patient care
Demonstrated knowledge and skills within primary care as well as theory and practice within a patient and family centred model of care
Demonstrated knowledge of relevant Professional Regulations and Standards (e.g., Community Health Nurse of Canada competencies and standards of practice)
Demonstrated knowledge and skill in the provision of evidence-based clinical practice related to primary care
Demonstrated ability to provide culturally safe care to diverse populations
Demonstrated knowledge and experience in Trauma Informed Practice, Harm Reduction, and Recovery Oriented Care

Responsibilities

Triage, assess, plan, implement, and evaluate nursing care for patients and families requiring high priority care within 24 hours

Apply chronic disease management, palliative, health management, and/or self-care management principles to conduct assessments and provide direct care
Address issues or variances from the plan of care
Act as a resource for the interdisciplinary team by providing information based on nursing theory and practice related to patients with primary level care needs
Practice in accordance with the British Columbia College of Nurses and Midwives standards of practice and VCH policies and decision support tools
