About this role
Within an interdisciplinary team-based care model, the Community Health Nurse cares for clients with acute, chronic and palliative care needs. Working in partnership with clients and families, the nurse establishes safe, realistic, and reasonable goals through comprehensive assessment and planning.
Day-to-day responsibilities include assessing service eligibility, facilitating the development and adaptation of care plans, delivering direct care, and providing care management. The nurse helps clients navigate community services and manage their own care, incorporating current evidence into practice.

Should there be insufficient qualified candidates, VCH may offer an 8-week training program. Candidates accepting this opportunity agree to remain with VCH in the applicable nursing area for at least 18 months after completing training, ensuring consolidation and application of skills in the specialty area.
Requirements
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One year recent related clinical experience in a relevant area working with complex clients with chronic health conditions and/or functional mobility issues.
- Recent experience in care and transition planning, or an equivalent combination of education, training, and experience.
- Valid BC Driver’s License required, as local area travel may require use of a vehicle or other accepted mode of transportation.
- Demonstrated knowledge and skills in community health nursing theory and practice within a client and family centred model of care.
- Demonstrated ability to provide client-focused, trauma-informed assessment and intervention.
Responsibilities
- Care for clients with acute, chronic, and palliative care needs within an interdisciplinary team-based care model.
- Assess service eligibility and facilitate the development, implementation, and adaptation of care plans.
- Deliver direct care and provide care management to assist clients in managing their own care.
- Work in partnership with clients and families to establish safe, realistic, and reasonable goals.
- Coordinate, organize, implement, evaluate, and transition nursing and other services for clients.
- Navigate clients through various community services and consult with system partners.
- Practice in accordance with BCCNM standards, Code of Ethics, and VCH policies and Decision Support Tools.
