About this role
CVS Health is seeking a Per Diem Clinician, either a Nurse Practitioner or Physician Assistant, to provide In-Home Health Evaluations for health plan members in Nevada. This role focuses on building trusted relationships and bridging gaps in care by meeting people in the comfort of their own homes.
Each visit lasts about 45 minutes, allowing time for one-on-one connection and answering health questions. You will conduct a medication and medical history review, a physical evaluation, and, if ordered, additional diagnostic tests such as diabetic eye exams or spirometry.
As an in-home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may include household pets, secondhand smoke, or other airborne allergens, so you must be comfortable and safe performing duties in varied conditions.
Clinicians enjoy the autonomy of working per diem with Signify Health, focusing solely on patient care without administrative burdens. You will have the time to provide education and truly listen to members' concerns, making a meaningful impact in communities across Nevada.
Requirements
- Active and unrestricted Nurse Practitioner or Physician Assistant license in the state of Nevada
- Ability to comfortably and safely perform duties in private residences that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
- Willingness to travel locally and within licensed states as needed
- Proficiency with using an iPad and clinical workflow software
- Strong clinical assessment and patient education skills
- Ability to work independently and manage a per diem schedule
Responsibilities

- Conduct in-home health evaluations including medication and medical history review and physical evaluation
- Perform additional diagnostic tests as ordered by the health plan, such as diabetic eye exams and spirometry
- Use an iPad and an intuitive clinical workflow for each evaluation
- Be flexible to travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide all Signify Health services including Diagnostic & Preventive Services products
