About this role

Within an interdisciplinary team-based care model, the Community Health Nurse provides care for clients with acute, chronic and palliative care needs in collaboration with clients, families and caregivers. The role focuses on establishing safe, realistic goals that support client independence and well-being across community settings.

Daily work involves assessing service eligibility, developing and adapting care plans, delivering direct care and coordinating services. The nurse evaluates client progress, manages transitions and incorporates current evidence into practice while following BCCNM standards and VCH policies.

The position operates within Vancouver Coastal Health's implementation of minimum Nurse to Patient Ratios, emphasizing quality practice environments. Nurses work across familiar and unfamiliar home environments, applying trauma-informed and culturally safe approaches with system partners.

Guided by values of care, continuous learning and better results, the role supports professional growth through complex client management and care coordination. Nurses develop expertise in community health theory while contributing to provincial initiatives that strengthen nursing practice.

Requirements

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
One year recent related clinical experience working with complex clients with chronic health conditions and functional mobility issues
Experience in care and transition planning or equivalent combination of education, training and experience
Valid BC Driver’s License required for local area travel
Demonstrated knowledge and skills in community health nursing theory and practice within a client and family centred model
Demonstrated ability to provide client-focused, trauma informed, harm reduction and culturally safe care
Demonstrated ability to assess safety and risk and integrate data from multiple sources to problem-solve effectively
Demonstrated ability to manage psychosocial and behavioral issues and employ effective conflict resolution approaches

Responsibilities

Care for clients with acute, chronic and palliative care needs within an interdisciplinary team-based model
Work in partnership with clients and families to establish safe, realistic and reasonable goals
Assess service eligibility and facilitate development, implementation and adaptation of care plans
Deliver direct care and provide care management to assist clients in navigating community services
Assess, plan, coordinate, organize, implement, evaluate and transition nursing and other services
Work in consultation with system partners and incorporate current evidence into practice
Practice in accordance with BCCNM standards, Code of Ethics and VCH Decision Support Tools
