Signify Health, part of CVS Health®, is seeking a Per Diem Clinician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes to help bridge gaps in care.

The visit lasts about 45 minutes, allowing time to connect one-on-one and answer health questions. It includes a medication review, physical evaluation, and additional diagnostic tests if ordered by the health plan. You'll use an iPad and intuitive clinical workflow, with no requirement to prescribe medicine or alter treatment regimens.

Your primary workplace is the private residence of health plan members. These environments are diverse and may present various conditions, including household pets, secondhand smoke, or other allergens. Candidates must be able to comfortably and safely perform their duties in these homes.

You'll play a critical role in building trusted relationships, bringing comprehensive health evaluations to people's homes. Join a national network of purpose-driven clinicians dedicated to improving health outcomes and simplifying health care.

Requirements

Current, unrestricted Nurse Practitioner or Physician Assistant license in the state of Nebraska.
Willingness to travel locally and within licensed states to members' homes.
Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens.
Proficiency with iPad and clinical documentation software.
Ability to perform comprehensive health evaluations and use diagnostic equipment as ordered.
Reliable transportation and a valid driver's license.

Responsibilities

Bring your heart into every visit, engaging with people in the comfort of their own homes.
Conduct 45-minute in-home health evaluations including medication review, physical evaluation, and diagnostic tests as ordered.
Use an iPad and intuitive clinical workflow for each evaluation.
Provide Diagnostic & Preventive Services including spirometry, diabetic eye exams, bone density screening, and other indicated tests.
Be flexible to travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Work with Signify Health as a clinician colleague, fulfilling the requirements of the role.
