As a Per Diem Clinician with Signify Health, you'll provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. Each visit lasts about 45 minutes, allowing time to connect one-on-one, answer health questions, and perform a medication and medical history review along with a physical evaluation.

You'll use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests, or alter people's current treatment regimen. Your primary workplace is the private residence of health plan members, which may include diverse environments with pets, secondhand smoke, or other airborne allergens.

Join a national network of purpose-driven clinicians dedicated to improving health outcomes. You'll have the flexibility to travel locally and within licensed states, conduct virtual visits as needed, and provide all Signify Health services including diagnostic and preventive screenings like spirometry and diabetic eye exams.

Requirements

Active Nurse Practitioner or Physician Assistant license in Nebraska
National board certification (e.g., ANCC, AANP, NCCPA)
Ability to travel locally within Scotts Bluff and surrounding areas
Comfortable performing duties in diverse home environments that may include pets, secondhand smoke, and other potential airborne allergens or irritants
Proficiency with iPad and electronic clinical documentation
Current BLS certification

Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
Be flexible to travel locally and within licensed states, as needed
Conduct virtual visits as needed
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
Perform in-home health evaluations including medication and medical history review, physical evaluation, and additional diagnostic tests as ordered
