About this role

Vancouver Coastal Health (VCH) is seeking a Community Health Nurse to join the Community Care Clinic team in North Vancouver, BC. Within an interdisciplinary team-based care model, you will care for clients with acute, chronic and palliative care needs. You will work in partnership with clients and families to establish safe, realistic goals.

Your day-to-day involves assessing, planning, coordinating, organizing, implementing, evaluating, and transitioning nursing and other services for clients. You deliver direct care and provide care management to help clients manage their own care and navigate available community services. You incorporate current evidence into practice.

You work in consultation with other system partners within an interdisciplinary team. You practice in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards, the Code of Ethics for Registered Nurses, and VCH's policies and Decision Support Tools.

Requirements

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
One year recent related clinical experience in a relevant clinical area working with complex clients/patients with chronic health conditions and/or functional mobility issues including recent experience in care and transition planning, or equivalent combination.
Valid BC Driver’s License required as local area travel may require the use of a vehicle.
Demonstrated knowledge and skills in community health nursing theory and practice within a client and family centred model of care.
Demonstrated ability to provide client-focused, trauma informed, Harm reduction, and culturally safe care in completing initial and ongoing client assessments and interventions.
Demonstrated ability to assess safety and risk prior to entering familiar and unfamiliar home environments and to integrate and evaluate pertinent data to problem-solve effectively.
Demonstrated ability to manage psychosocial and behavioral issues and employ effective conflict resolution and reconciliation approaches related to goals of care, transitions and end of life care.
Demonstrated ability to manage and prioritize clients and promote cooperation, assertiveness, creative planning for change and innovations.

Responsibilities

Care for clients with acute, chronic and palliative care needs within an interdisciplinary team-based care model.
Assess service eligibility and facilitate the development, implementation, and adaptation of individualized care plans.
Deliver direct care and provide care management to assist clients in managing their own care.
Work in partnership with clients and families to establish safe, realistic, and reasonable goals.
Coordinate and navigate clients through various community services and system partners.
Incorporate current evidence into practice in accordance with BCCNM standards and VCH's Decision Support Tools.
Provide client-focused, trauma informed, harm reduction, and culturally safe care during assessments and interventions.

Benefits

Salary range of CAD $41.42/Hr. - CAD $55.91/Hr.
