About this role

Within an interdisciplinary team-based care model, the Community Health Nurse cares for clients with acute, chronic and palliative care needs in partnership with clients, families, and caregivers. This role focuses on establishing safe, realistic, and reasonable goals through comprehensive assessment, planning, and coordination of services.

Day-to-day responsibilities include assessing service eligibility, developing and adapting care plans, delivering direct care, and providing care management to help clients navigate community services. The nurse also facilitates transitions and incorporates current evidence into practice to support client self-management.

The Community Health Nurse works in consultation with system partners and operates within the standards of the British Columbia College of Nurses and Midwives (BCCNM) and VCH values. This position is part of the provincial implementation of minimum Nurse to Patient Ratios (mNPRs), an initiative aimed at enhancing nursing practice and strengthening quality of care.

Requirements

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
One year recent related clinical experience in a relevant area working with complex clients with chronic health conditions and/or functional mobility issues.
Recent experience in care and transition planning, or an equivalent combination of education, training, and experience.
Valid BC Driver’s License required for local area travel using a vehicle or other accepted mode of transportation.
Demonstrated knowledge and skills in community health nursing theory and practice within a client and family centred model of care.
Demonstrated ability to provide client-focused, trauma-informed, harm reduction, and culturally safe care in assessments and interventions.
Demonstrated critical thinking to assess safety and risk in home environments and integrate data from multiple sources to problem-solve effectively.
Demonstrated ability to manage psychosocial and behavioral issues and employ conflict resolution and reconciliation approaches related to goals of care and transitions.

Responsibilities

Care for clients with acute, chronic and palliative care needs within an interdisciplinary team-based care model.
Work in partnership with clients and families to establish safe, realistic, and reasonable goals.
Assess service eligibility and facilitate the development, implementation, and adaptation of care plans.
Deliver direct care and provide care management to assist clients in managing their own care.
Coordinate and transition nursing and other services to help clients navigate community settings.
Incorporate current evidence into practice and consult with system partners as needed.
Practice in accordance with BCCNM standards, Code of Ethics for Registered Nurses, and VCH policies and Decision Support Tools.
