About this role

This role provides direct and indirect patient care services to individuals in their home environment or alternative setting, following the organization's policies and procedures. The Behavioral Health Nurse assesses patients and develops a plan of care in collaboration with the physician, delivering skilled nursing interventions as ordered.

Day-to-day responsibilities include managing medical illness per the plan of care, performing psycho-biological interventions, and conducting comprehensive medication reviews. The nurse also provides education to patients and families on medication regimens, administers intramuscular or intravenous medications, and manages situational or other crises, including assessments of potential self-harm or harm to others.

Working within a collaborative care team, the nurse supervises and directs Licensed Practical Nurses and Home Health Aides involved in the patient's plan of care. The role also involves coordinating with other disciplines to ensure collaboration and coordination of care, and assisting with community education and marketing activities when requested.

This position offers the opportunity to provide supportive counseling psychotherapy and psycho-therapeutic interventions according to education and licensure, as well as ongoing evaluation and management of the patient's care plan. The nurse promotes patient independence and rights while ensuring a safe and therapeutic environment.

Requirements

Licensed professional nurse with current state licensure
Ability to assess patients and develop a plan of care in collaboration with a physician
Experience with psycho-biological interventions and crisis management
Proficiency in medication administration including intramuscular and intravenous routes
Skilled in providing patient and caregiver education on disease process, symptom management, and safety
Ability to supervise and direct Licensed Practical Nurses and Home Health Aides
Knowledge of state, local, and payer requirements for documentation and care standards

Responsibilities

Makes initial assessment visit and assists in developing the patient's plan of care in coordination with the physician and Clinical Manager
Manages medical illness per plan of care and performs psycho-biological interventions
Conducts comprehensive medication reviews and educates patients and caregivers on medication regimen, including frequency, duration, side effects, and contraindications
Administers intramuscular or intravenous medications as ordered by the physician
Manages situational or other crises and performs assessments of potential self-harm or harm to others, referring to treating physicians as necessary
Provides supportive counseling psychotherapy and psycho-therapeutic interventions according to education and licensure
Provides general nursing interventions including phlebotomy, wound care, infusion, ostomy, and catheter care as ordered by the physician
Ensures appropriate documentation in accordance with coverage guidelines and completes documentation within required timeframe
