About this role

This role is responsible for identifying patient, family, and facility care needs for terminally ill patients in Prince George's County. Reporting to the Team Manager, you will provide comprehensive nursing and supportive care to ensure the highest quality of life for patients and their families.

Day-to-day, you will complete initial assessments of patients and families to ascertain home care needs, including physical, emotional, spiritual, and mental status. You will prepare and implement care plans in collaboration with the Interdisciplinary Group, teach family members and facility staff patient care skills such as pain control, medication management, and wound care, and attend to patients and families at the time of death.

Working within a hospice care team, you will collaborate with the Clinical Manager in utilizing team members, outside agencies, and community resources. You will document in facility charts, reconcile medications, and maintain positive, open communication with facility staff to ensure coordinated care.

This position offers the opportunity to develop specialized expertise in hospice and palliative nursing within a supportive healthcare organization. You will grow your skills in complex symptom management, interdisciplinary collaboration, and end-of-life care coordination.

Requirements

  • Diploma, ADN and/or BSN from an accredited institution
  • Current Maryland State license as a Registered Nurse
  • Two years of nursing experience, or equivalent combination of education and experience
  • Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection
  • Strong time management skills
  • Excellent interpersonal and communication skills
  • Ability to be patient and family focused
  • Skill in the use of Electronic Medical Record (EMR) and/or ability to learn

Responsibilities

  • Complete initial assessment of patient and family to ascertain home care needs, including physical, emotional, spiritual, and mental status
  • Prepare and implement the care plan for patient/family unit in collaboration with the Interdisciplinary Group
  • Provide direct nursing care to patients to maintain the highest level of care in conjunction with the primary physician
  • Teach family members, facility staff, and hired caregivers patient care skills including pain control, medication titration, injections, blood draws, IV management, dressing changes, and vital sign monitoring
  • Reconcile medications against facility medication list and update facility staff and family of visit schedule
  • Attend care plan meetings and patient care rounds for hospice patients within the facility
  • Ensure arrangements for necessary equipment in the facility setting are made available
  • Participate in marketing and customer service activities with Business Development Managers
