The Clinical Wound Care Nurse delivers consultation and specific clinical care to inpatients needing instruction and management for skin, wound, ostomy care, and incontinence. Treatment plans are developed and documented along with assessments, interventions, and outcomes in the electronic medical record.

Staff nurses receive education on skin care, wound care, pressure wound staging, and ostomy care to build independent practice within their scope. Patterns of wounds are analyzed across programs to target performance improvement activities that reduce pressure injuries.

Collaboration with the Clinical Education Department establishes wound care competencies and patient teaching tools. Input is provided on policies and procedures while maintaining current knowledge in wound, ostomy, and continence therapy across both main campuses.

Requirements

  • Bachelor’s degree in related field, BSN preferred
  • Must acquire CWOCN, CWON, or CWCN within one year of hire date
  • Wound Specialty Education from accredited program (WCC, WTA, CWS, or CWCA) preferred
  • Two years of experience working as a registered nurse in an acute care healthcare facility or ambulatory wound care center
  • AHA BLS - Healthcare Provider
  • Registered Nurse – NJ or Registered Nurse – NLC multi-state

Responsibilities

  • Provide consultation and specific clinical care to inpatients needing instruction, management, and nursing treatment plans for skin, wound, ostomy care, and incontinence management.
  • Document assessments, plan of care, interventions, evaluation, outcomes, and nursing orders on appropriate chart forms and the inpatient electronic medical record.
  • Assess need for patient education, plan and implement education, and evaluate and document patient comprehension and compliance.
  • Educate staff nurses in skin care, wound care, pressure wound staging and care, and ostomy care to encourage independent practice within scope.
  • Identify potential discharge needs and notify Case Management or other services to arrange follow-up care and transition to the next level of care.
  • Analyze patterns of wounds across programs and hospitals and identify targeted performance improvement activities to improve outcomes and mitigate pressure injuries.
  • Work with Clinical Education Department to establish competencies for clinicians through collaborative educational modules and patient teaching tools.
  • Provide input and feedback for wound and ostomy care policy and procedures while maintaining knowledge base in wound, ostomy, and continence therapy.
