About this role

The Primary Care Nurse delivers comprehensive nursing care to patients across the lifespan in a primary care setting. Working at the top of licensure, the nurse supports planned care models and chronic disease management while coordinating with providers for optimal patient outcomes.

Daily responsibilities include administering medications and vaccines, performing wound care and specimen collection, and providing targeted health education. The nurse monitors patient responses, documents all care accurately in the electronic health record, and ensures infection control standards are maintained.

As part of a clinical team, the nurse collaborates with primary care providers and community-based organizations to coordinate care and support transitions. This includes following up on hospital discharges and acting as a resource for medical assistants on procedures and patient preparation.

The position applies principles of advanced access and rapid cycle improvement using the PDSA approach. Nurses maintain compliance with mandatory trainings and oversee supplies while building expertise in electronic health records for confidential patient information management.

Requirements

  • Bachelor of Science in Nursing (BSN) or Associates of Science in Nursing (ASN)
  • Expert use of electronic health record for timely and confidential patient information management
  • Ability to monitor patient response to medications and respond to any adverse reactions
  • Knowledge of advanced access, planned care, and chronic care model principles
  • Compliance with agency mandatory annual training requirements including Patient Safety and Age Based Competencies

Responsibilities

  • Support primary care of patients under the planned care model in collaboration with PCPs to identify preventive and chronic disease needs
  • Provide general and specific health care education and work with patients to identify and monitor self-management goals
  • Administer medications, vaccines, and immunizations orally, subcutaneously, intramuscularly, or via nebulizer per provider orders
  • Ensure safe, accurate collection, storage, and processing of all biologic specimens while adhering to documentation protocols
  • Perform nursing activities such as wound care and catheterization after initial prescription by the primary care provider
  • Accurately document all aspects of patient care in the medical chart on a timely basis and schedule specialty referrals
  • Maintain strict infection control standards in accord with CHCI protocols, OSHA, and CLIA requirements
