About this role
At Northwestern Medicine, the Staff Nurse delivers care that reflects the organization's mission and values while adhering to the Code of Ethics and all regulatory standards. This clinically competent registered nurse prescribes, delegates, and coordinates nursing care for acutely ill patients in the home setting.
Daily work includes assessment, planning, intervention, implementation, and evaluation through the nursing process. The nurse manages phone triage, delivers abnormal test results, and provides corresponding care instructions while maintaining ANA standards of professional practice.
The role requires effective interaction with patients, families, and the broader health team. Hours are Monday through Friday, 8:00am-4:30pm, with no holidays, weekends, or on-call requirements, supporting a predictable schedule in the home care environment.

Requirements
- Current experience within the past five years or new nursing graduate within the past two years
- State Licensed Registered Nurse for State of employment
- CPR – Basic Life Support
- Bachelors of Science in Nursing
- One year experience in pertinent clinical specialty
- Proficiency in second language
Responsibilities
- Perform all aspects of patient care by utilizing evidenced based practice in a manner which optimizes patient safety and reduces the likelihood of medical/health care errors.
- Follow the six medication rights and reduce the potential for medication errors, administering medications based on the age and clinical condition of the patient.
- Assess, analyze, validate and integrate data to assess patients' physical, psychosocial, rehab, safety, learning, spiritual, cultural, age-specific, special risk and self-care needs.
- Develop, implement and update an individualized plan of care based on identified patient care needs, including nursing, teaching, continuum of care and discharge planning.
- Adequately assess and reassess pain, utilize appropriate pain management techniques, and educate the patient and family regarding pain management.
- Identify the patient's learning needs, abilities, preferences and readiness, and explain procedures, therapies and treatments to patient and family.
- Participate in shared governance, committee work, and practice/department projects while using quality improvement results to initiate changes in nursing practice.
