About this role

The primary purpose of the Integrated Care Nurse is to serve patients in the serious mental illness intensive outpatient program as part of an integrated health team. This role provides care assessment and promotes restoring a patient's mental and physical health through collaboration with physicians, nurse practitioners and multidisciplinary healthcare team members.

The Integrated Care Nurse ensures delivery of whole health care including collaboration and referral to community social service and behavioral health supports. The position requires substantial specialized knowledge, judgement and nursing skills based upon the principles of psychological, biological, physical, and social sciences in the application of the nursing process.

Daily work includes completing patient assessments, formulating and overseeing plans of care, leading educational groups, and performing medication administration. The nurse supervises LPNs, CNAs, clerks and students while maintaining accurate electronic records in compliance with HIPAA.

The role serves as patient advocate during Treatment Team and discharge planning, coordinates care during medical or psychiatric emergencies using SBAR, and assists patients with activities of daily living. The nurse mentors staff, participates in performance improvement initiatives and assumes responsibility for professional development.

Requirements

  • Substantial specialized knowledge and nursing skills based on psychological, biological, physical and social sciences
  • Ability to apply the nursing process with critical thinking skills to assess, implement and evaluate care plans
  • Experience supervising LPNs, CNAs, unit clerks and students in a behavioral health setting
  • Proficiency in electronic patient records and strict adherence to HIPAA and confidentiality policies
  • Skill in leading educational groups and providing self-management education for serious mental illness
  • Competence in SBAR communication and coordination during medical or psychiatric emergencies
  • Ability to assist patients with activities of daily living and serve as a compassionate advocate

Responsibilities

  • Completes patient assessments and consultation to obtain psychological history and risk factors and formulates plan of care
  • Develops care plans in collaboration with the patient and care team that provide support and referrals to achieve goals
  • Leads educational groups and individual nursing education on self-management skills for mental health and behavioral health conditions
  • Performs, supervises, delegates and oversees medication administration, patient observations and other nursing skills by LPNs, CNAs and students
  • Mentors and precepts students and orients new staff as assigned
  • Participates in Treatment Team, discharge planning and transition of care
  • Serves as patient advocate, escalates concerns timely and practices Hands Off communication utilizing SBAR during emergencies
  • Maintains accurate electronic patient records in compliance with HIPAA and documents pertinent findings in the medical record
