About this role

Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities. This is achieved through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff. The role focuses on primary-care services in long-term care settings.

Provide primary-care services in accordance with site-specific protocols, consistent with family medicine, geriatric medicine, or internal medicine standards for long-term care. Review and sign MARs, orders, labs within 24 hours, and manage narcotic prescriptions per policy. Complete documentation and submit billing records within published timelines.

Establish and maintain open communications with facility staff and administration, meeting daily with administrators or care coordinators. Adhere to Eventus WholeHealth standards, policies, procedures, and all regulatory guidelines. Maintain patient confidentiality abiding by HIPAA rules.

Consult with clinical supervisor before refusing resident, family, or facility requests. The part-time role supports facility patient care needs with adequate time for administrative duties each visit. Benefits go beyond the basics to support providers caring for others.

Requirements

  • Masters degree in Nursing from an accredited educational institution and a current and valid license to practice as Nurse Practitioner issued by the state in which you reside Medical Board
  • All other certification necessary to perform one’s duties must be current and valid
  • OR graduated from a physician assistant educational program that is accredited by a National Commission on Accreditation of Allied Education Programs
  • OR have passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants
  • OR licensed by the state in which you reside as a Physician Assistant
  • Specialization in geriatric, family, or psychiatry desired

Responsibilities

  • Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and consistent with the standard of care for family medicine, geriatric medicine, and/or internal medicine as it pertains to long term care facility work
  • Review and sign MAR’s as requested by facilities, sign all orders specific for Primary Care while at the facility and/or within 24 hours
  • Review and address labs within 24 hours of them being sent and manage all narcotic prescriptions specific to facility’s patients per Eventus WholeHealth Narcotic Rx Policy
  • Complete documentation and submit billing records to Eventus WholeHealth according to published policies on charting and bill sheet submission
  • Establish and maintain open and positive communications with facility staff and administration and meet with Administrator or designated facility care coordinator each day
  • Be knowledgeable of and adhere to Eventus WholeHealth standards, policies and procedures and comply with all regulatory agencies governing health care delivery
  • Maintain patient confidentiality at all times including appropriate use of cell phone, emails, text messaging, patient charts and EMR and abide by HIPAA rules
