About this role
Keystone Human Services seeks an Agency Nurse (Health Services Consultant) to join professionals making a positive difference in the community. This role provides leadership and guidance to program support teams, contracted providers, and individuals to ensure quality care for adults with intellectual disabilities.
Work collaboratively with the interdisciplinary team and outside organizations, including community health providers and external stakeholders. Address medical needs by implementing, monitoring, and coordinating healthcare services. Adhere to Keystone’s mission and regulatory requirements throughout all activities.
Engage daily with program support teams and individuals receiving services. Coordinate care across multiple providers to meet diverse health needs. Maintain compliance with health service standards in a supportive team environment.

Requirements
- Valid PA Registered Nurse (RN) or Licensed Practical Nurse (LPN) license
- Three years of work experience in the medical field, preferably with individuals diagnosed with intellectual and developmental disabilities
- Ability to communicate both orally and in writing and to read, interpret and apply written information
- Proficient computer skills, including the use of software and web-based systems
- Valid driver’s license and access to properly insured vehicle
- Successful completion of the pre-employment process, including motor vehicle record and criminal background checks
