In this role, you will visit homes and long-term care facilities to provide treatment and support to residents. You will monitor and manage stable conditions through regular in-home visits, offering daily or weekly follow-up care. Comprehensive in-home assessments are conducted for patients unable to attend clinic appointments.

Requirements

  • Graduate from an accredited School of Nursing
  • Licensed Nurse registered in good standing with the governing body
  • Willing to obtain a nursing license through the regulating body in the provinces or territories you wish to work
  • Meet required work experience (details discussed during interview)
  • Valid CPR certification (BLS for Healthcare Providers)
  • Ability to travel and work in remote northern communities

Responsibilities

  • Visit homes and long-term care facilities to provide treatment and support for residents as needed
  • Monitor and manage stable conditions through regular in-home visits, offering daily or weekly follow-up care
  • Conduct comprehensive in-home assessments and deliver ongoing care for patients unable to attend clinic appointments
  • Provide direct nursing care including wound care, medication administration, and patient education
  • Coordinate with local health teams and family members to ensure continuity of care
  • Document patient assessments and care plans accurately
