We are looking for a compassionate Licensed Practical Nurse (LPN) to join our team. You will accurately assess, communicate, and document residents' status, observing and reporting significant changes to the Resident Care Director, physician, family, RCAs, and Executive Director. You will direct and supervise the daily work assignments of Resident Care Assistants and closely mentor Lead Resident Care Assistants.

Requirements

  • Current state license as a Licensed Practical Nurse (LPN).
  • CPR certification.
  • Graduate of an approved LPN program (per state requirements).
  • Minimum of 1 to 2 years of experience working as a nurse preferred.
  • Previous experience working with the elderly in a paid or volunteer position is highly desirable.
  • Ability to supervise and direct daily work assignments of Resident Care Assistants.
  • Knowledge of state regulations regarding elder abuse, neglect, and exploitation reporting.
  • Strong assessment and documentation skills.

Responsibilities

  • Accurately assess, communicate, and document residents' status.
  • Observe and report significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director per state regulations.
  • Direct and supervise the daily work assignments of Resident Care Assistants.
  • Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their job.
  • Coordinate care needs with community providers via an effective case management process.
  • Effectively balance service demands with supporting resident independence.
  • Recognize and assist in the prevention of elder abuse, neglect, and exploitation, reporting to appropriate sources per state regulations.
  • Respond to emergencies and personal emergency response system, knowing when to call for backup.
