About this role

Adoration Home Health and Hospice, a part of BrightSpring Health Services, is seeking a Behavioral Health Nurse to provide direct and indirect patient care in the home environment. This licensed professional assesses patients and develops a plan of care in collaboration with the physician, delivering skilled nursing interventions including phlebotomy, wound care, infusion, ostomy, and catheter care.

Day-to-day responsibilities include managing medical illness per the plan of care, performing psycho-biological interventions, and conducting comprehensive medication reviews. The nurse educates patients and caregivers on medication regimens, side effects, and compliance, while also managing situational crises and assessing for potential self-harm or harm to others.

This role involves supervising and directing Licensed Practical Nurses and Home Health Aides involved in the patient plan of care. The nurse collaborates with other disciplines to ensure coordination of care, provides supportive counseling and psycho-therapeutic interventions according to licensure, and promotes a safe, therapeutic environment that encourages patient independence.

Requirements

  • Licensed professional nurse with current state licensure
  • Experience in behavioral health or psychiatric nursing
  • Ability to perform comprehensive patient assessments and develop plans of care
  • Proficiency in medication management and patient education
  • Skilled in crisis intervention and assessment of self-harm or harm to others
  • Competence in general nursing interventions including phlebotomy, wound care, infusion, and catheter care
  • Ability to supervise and direct Licensed Practical Nurses and Home Health Aides
  • Strong documentation skills to meet company, regulatory, and payer standards

Responsibilities

  • Makes initial assessment visit utilizing observation/assessment skills and assists in developing the patient's plan of care in coordination with the physician and Clinical Manager
  • Manages medical illness per plan of care and performs psycho-biological interventions
  • Conducts comprehensive medication reviews and educates patients and caregivers on medication regimen, frequency, duration, side effects, and contraindications
  • Manages situational or other crises and performs assessments of potential self-harm or harm to others, referring to treating physicians as necessary
  • Provides supportive counseling psychotherapy and psycho-therapeutic interventions according to education and licensure
  • Provides general nursing interventions including phlebotomy, wound care, infusion, ostomy, catheter care, and other skilled interventions as ordered by the physician
  • Monitors the patient's condition and reports changes to the Clinical Manager and the patient's physician, ensuring appropriate documentation per coverage guidelines
  • Participates with after-hours coverage to ensure continuity of care
