The Nurse Practitioner provides assessments, disease and treatment education, and symptom management for patients facing chronic, complex, and life-threatening conditions. Care is delivered in collaboration with the PCP and specialists to help patients and families maintain maximum comfort and coping at various locations.

Daily work includes performing routine and emergency assessments related to outpatient palliative care visits in the patient's place of residence. The practitioner orders and interprets laboratory and radiology tests, prescribes medications including controlled substances, and orders treatments and durable medical equipment within scope of practice.

The position requires flexing the schedule to meet agency visit needs and collaborating appropriately with the assigned Compassus Medical Director. Compliance with practice standards, codes, and Collaborative Agreements is maintained while initiating reimbursement for services rendered.

Requirements

  • Licensed to prescribe medications including controlled substances
  • Complies with applicable laws and regulations with respect to Collaborative Agreements
  • Maintains accurate records to support reimbursement for services provided

Responsibilities

  • Performs routine and emergency assessments related to outpatient palliative care visits
  • Performs bedside palliative care patient/family evaluation in the patient's place of residence
  • Assists with diagnosis, treatment, and management of acute and chronic health conditions
  • Orders, performs, and interprets laboratory and radiology tests within scope of professional practice
  • Prescribes medications including controlled substances to the extent delegated and licensed
  • Orders treatments and durable medical equipment as indicated
  • Consults with palliative care physician or designees as needed and informs primary physician of services provided
  • Prepares and maintains accurate patient records, charts, and documents to support sound medical practice
