About this role

The Licensed Practical Nurse furnishes comprehensive client care as assigned by the Case Manager in accordance with the Nurse Practice Act, professional principles, and company policies. This role demands substantial specialized knowledge, judgement, and skill from psychological, biological, and social sciences.

This full-time position involves Monday-Friday shifts with on-call rotation, traveling to client homes for 1:1 care lasting 1-3 hours per visit. Coverage spans Mendota Heights, Bloomington, Minneapolis, St. Paul, Cottage Grove, and surrounding areas. Nurses plan, provide, and evaluate care while administering medications and treatments as ordered.

The LPN recognizes symptoms, institutes remedial measures within Minnesota legal limits, and reports changes to the Case Manager. Effective communication occurs with the interdisciplinary team, including participation in client conferences. Accurate documentation and maintenance of clinical records ensure timely follow-through.

Requirements

  • Licensed Practical Nurse with Minnesota LPN license and adherence to Nurse Practice Act
  • Substantial specialized knowledge, judgement and skill based on psychological, biological and social sciences
  • Ability to travel to client homes in Mendota Heights, Bloomington, Minneapolis, St. Paul, Cottage Grove and surrounding areas
  • Capable of driving or using public transportation in all types of weather
  • Physical ability for medium to very heavy work, exerting in excess of 100 pounds
  • Adaptable to a variety of home environments and community settings
  • Exercises professional judgement in adapting nursing measures to changing needs

Responsibilities

  • Plans, provides and evaluates nursing care in accordance with established policies and procedures
  • Recognizes and interprets symptoms and institutes remedial measures within legal limits of LPN nursing
  • Documents observations and teachings completely, accurately, legibly and concisely
  • Administers medications and treatments as ordered by the physician and delegated by the Case Manager
  • Communicates effectively with all members of the interdisciplinary team
  • Participates in scheduled client conferences and maintains accurate clinical records
  • Assists client in arranging medical appointments and contacts with community resources
  • Participates in the after-hours on call process to ensure client care
