RN Shift Nurse - Home & School Care

Work as a Shift Nurse providing care to clients in home or school settings, including pediatric patients. Help patients of all ages with differing complexity of care needs remain comfortable in their homes for as long as possible. Assess patients’ individualized needs using appropriate tools and techniques. Execute and continuously monitor care plans while engaging patients collaboratively to build rapport and trust. Maintain and update professional patient records related to encounters and plans.
