About this role

This full-time role coordinates nursing and hospice aide staff for day-to-day continuity of care in Hospice Home. It supervises assigned shift Hospice Home RNs, LPNs, and aides, with intermittent oversight of interdisciplinary team members. The schedule runs Monday-Friday 10:30pm-7:00am, plus weekends and holidays as needed.

Supervise, direct, prioritize, and coordinate patient care delivery on the assigned night shift. Ensure admissions are assigned appropriately and perform weekly chart audits for documentation compliance. Coordinate and problem-solve care issues with patients, families, physicians, and staff.

Create a positive environment fostering collaboration and support among team members. Demonstrate strong communication, problem-solving, organizational, and delegation skills. Manage personnel actions like hiring, orientation, appraisals, scheduling, and discipline.

Oversee medication room operations, clinical compliance, and regulatory needs. Develop processes for quality improvement and update policies as needed. Identify staff competency needs and cover management backup plus at least one holiday annually.

Requirements

  • RN with current N.C. license
  • Two years of previous supervisory experience preferred
  • Current CPR certification
  • Thorough understanding of Hospice clinical interventions and protocols
  • Knowledgeable about reimbursement sources, Medicare/Medicaid regulations, licensing laws and accreditation standards
  • Excellent written and verbal communication skills
  • Strong team building skills and leadership abilities
  • Competent in use of computer, able to learn and teach use of new software programs

Responsibilities

  • Supervise, direct, prioritize and coordinate delivery of patient care for Hospice Home patients on assigned shift
  • Coordinate interviewing, hiring and orientation of new clinical staff
  • Perform chart audits weekly to ensure compliance with documentation requirements
  • Coordinate and problem-solve patient care issues with patients, families, physicians and agency staff
  • Ensure staff documentation of patient care is complete, timely and accurate
  • Manage personnel actions including hiring, orientation, performance appraisals, promotions, scheduling and discipline
  • Oversee medication room operations and regulatory compliance related to narcotics and other medications
  • Develop and implement processes to enhance clinical operations and ongoing quality improvement
