The Visit Registered Nurse assesses and identifies patient/family needs to plan, implement, evaluate, and document individualized hospice nursing care per the interdisciplinary plan. They handle visits in homes, skilled nursing facilities, assisted living facilities, or acute care settings based on team coverage needs. This includes unplanned visits, follow-ups on new admissions, and conducting admission visits.

Visit Nurses coordinate care and collaborate among the Interdisciplinary Group and other medical providers. They supervise hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants. Excellence in nursing practice supports the organization’s needs for all patients.

Requirements

RN with current RN licensure in the state of Kentucky and/or Indiana as required, or ability to obtain upon offer
BSN desired
Certification in hospice/palliative care preferred
One year of nursing experience preferred
Knowledge of palliative care helpful
Valid driver’s license
Use of technology-based tools, including electronic medical record, strongly preferred

Responsibilities

Assess and identify patient/family needs
Plan, implement, evaluate, and document individualized hospice nursing care in accordance with the interdisciplinary plan of care
Coordinate care and collaborate among the Interdisciplinary Group and other medical providers
Supervise hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants
Conduct visits to home patients for team coverage needs, unplanned visits, or follow-ups on new admissions
Perform admission visits for medically eligible patients into Hosparus Health care
Make facility to home admissions as needed
