Giving Home Health Care has supported individuals with work-related health conditions from Department of Energy nuclear facilities since 2012. The organization delivers personalized in-home care across nine states with no expiration on patient benefits once accepted into the program.

In this 1099 contract role you will administer care according to a physician-established plan at the patient’s residence in Youngsville. Visits total eight hours per month and are scheduled on Mondays or Tuesdays to suit both parties.

You will advocate for patients one-on-one while collaborating with CNAs, HHAs, case managers, and physicians. The arrangement supports long-term relationships that can continue for years with the same individuals.

Background check clearance is required before offers are finalized. The position offers the chance to provide consistent, mission-driven support within a patient’s home environment.

Requirements

  • Graduate of an accredited Associate or Baccalaureate School of Nursing.
  • Current State or Compact License as a Registered Nurse or Licensed Practical Nurse.
  • Current CPR certification.
  • Current TB test and physical examination.
  • Maintain required insurance and dependable transportation.
  • Ability to stand or walk for prolonged periods and lift up to 50 lbs.
  • Ability to provide basic medical management up to medically fragile end-of-life care.

Responsibilities

  • Execute physician prescribed plans of care.
  • Complete compliant documentation of the care provided in the system of record.
  • Manage and administer prescribed medication, treatment, and therapies.
  • Conduct patient assessments and coordination of care.
  • Monitor vitals, GI intake and output, and skin integrity including wound care.
  • Communicate with family members, physicians, and case managers regarding patient health.
  • Provide education and training to the patient, family, and Home Health Aide.
  • Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
