About this role
As an Advanced Practice Clinician, you will provide comprehensive primary care services within skilled nursing and senior living communities. This role focuses on delivering integrated, evidence-based care that improves patient outcomes.
Your day-to-day includes routine rounding, proactive chronic disease management, and coordination of care across interdisciplinary teams. You will work closely with facility partners to implement a value-based approach emphasizing preventive care and improved quality outcomes.

Requirements
- Current NP or PA licensure in the state of practice
- Previous professional patient care experience
- Experience in geriatrics, family medicine, or internal medicine preferred
- Exceptional patient-centered service skills including compassionate communication and active listening
- Strong collaboration skills with ability to work effectively across interdisciplinary teams
- Ability to build trusting relationships with residents, families, and facility staff
