Oak Hill is seeking a highly motivated and dynamic Registered Nurse (RN) to maintain and improve the health and wellbeing of individuals with Developmental and Intellectual Disabilities. The Substitute Group Home Nurse Coordinator works as needed, ensuring provision of all health services for program participants and monitoring implementation of physicians' and dentists' orders. This maintains their health and well-being in keeping with Oak Hill’s mission. The role expands ability to identify and address diverse community needs.

Interact daily with program participants to understand their needs, likes, dislikes, and personalities while developing familiarity with individual health issues. Collaborate with Manager, Program Supervisor, and direct care staff for proper health maintenance. Provide personalized service through ongoing assessments and staff training.

Requirements

  • Current Connecticut Registered Nurse license
  • Current Connecticut driver’s license
  • Ability to monitor implementation of physicians' and dentists' orders for individuals with developmental and intellectual disabilities
  • Proficiency in medication administration, storage, handling, and record-keeping
  • Experience assessing health status and providing treatments for health maintenance
  • Knowledge of training staff on health issues, seizure control, and hygiene
  • Familiarity with DDS CUP recertification and medication certification processes
  • Ability to review medical records, lab reports, and health information for accuracy

Responsibilities

  • Assess and monitor the health status of program participants on an ongoing basis, following physician orders with assistance from Manager, Program Supervisor, and direct care staff
  • Interact with program participants on an ongoing basis to understand their needs, likes, dislikes, personalities, and individual health issues
  • Write quarterly and annual health care summaries and submit to Program Supervisor before OPS/IDT meetings
  • Train staff and program participants in health-related issues including safe storage, handling and administration of medications, seizure control, and hygiene
  • Check medication log and medication cabinets regularly, noting and acting on errors, labels, expiration dates, and discontinued meds
  • Review and initial all incoming medical/health related information including physicians'/dentists' notes, lab reports, and special tests
  • Administer practical portions of med certification program, including Lab Practicum and On-Site med passes, and provide DDS-mandated CUP recertification training
  • Review medical records and take appropriate action when errors are found, and assist in developing self-administration of meds programs
