About this role

Porter is expanding throughout Maryland and is looking for Nurse Practitioners. Choose your county coverage—must be willing to cover 3 or more counties including Baltimore County, Baltimore City, Frederick, and others. Minimum 100 hours per month of availability preferred, with days available 7 days a week from 8a-8p.

You will visit members within your geographic area and conduct in-home health risk assessments. This includes evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams. Evidence-based screening involves point-of-care testing, identifying diagnoses for care management, and formulating lists of medical conditions.

Requirements

  • Valid unencumbered Maryland NP license
  • 3 years of patient care experience required
  • Experience in primary care, adult/geriatric preferred
  • EMR skills preferred
  • Strong sense of empathy, compassion, and friendliness
  • Exceptional communication skills, both written and oral
  • Willing to cover 3 or more counties
  • Minimum 100 hours per month of availability preferred

Responsibilities
