As a Behavioral Health Nurse, you will provide direct patient care including assessments and care plan development. You will manage a caseload, coordinate care across the clinical team, and administer medications per physician orders. Monitor patient progress and adjust care plans as needed while educating patients and caregivers on self-management.

You will communicate with physicians and care teams to ensure continuity of care, and supervise HHAs and LPNs. The patient-centered care model allows you to spend meaningful time with patients and truly make a difference. A supportive structure is designed to reduce burnout and promote well-being.

Requirements

  • Active RN license
  • Graduate of an accredited nursing program (BSN preferred)
  • Home health or related behavioral health clinical experience
  • Strong communication, organization, and critical thinking skills
  • Current CPR certification
  • Valid driver’s license and reliable transportation

Responsibilities

  • Provide direct patient care including assessments and care plan development
  • Manage a caseload and coordinate care across the clinical team
  • Administer medications and treatments per physician orders
  • Monitor patient progress and adjust care plans as needed
  • Educate patients and caregivers on care and self-management
  • Communicate with physicians and care teams to ensure continuity of care
  • Supervise and support HHAs and LPNs
  • Complete accurate and timely documentation including OASIS, visit notes, and orders
